Answers About Mobile Wound Care & Heart Failure Support
We know that managing both heart failure and chronic wounds can feel overwhelming for patients, caregivers and healthcare providers. Below are answers to some of the most common questions we receive about our services, treatments, and mobile care approach.
What areas do you serve?
We provide mobile wound care throughout Chicago and surrounding suburbs. Our clinicians come directly to homes, skilled nursing facilities, assisted living communities and rehab centers.
How does mobile wound care work?
Our licensed wound care clinicians travel to where the patient lives or resides. We evaluate, treat and manage chronic wounds at the bedside, no transportation or clinic visits required. All care is coordinated with the patient’s primary providers to ensure seamless support.
Why do you focus on patients with heart failure?
Patients with chronic heart failure have poor circulation, fluid retention and weakened immune response, all of which make wound healing more difficult. We specialize in this overlap, offering targeted therapies that improve outcomes for this high-risk population.
What types of wounds do you treat?
We treat wounds such as:
Arterial ulcers
Venous leg ulcers
Diabetic neuropathic ulcers
Pressure injuries (bedsores)
Post-surgical wounds
Wounds caused by swelling or poor circulation
What are CAMPs and CPTs?
CAMPs (Cellular and Matrix-Based Products) and CPTs (Current Procedural Therapies) are advanced biologic treatments that stimulate tissue repair and accelerate healing. These are especially helpful in hard-to-heal wounds, which are common in heart failure patients.
Who provides the care?
All care is delivered by licensed clinicians trained in wound care and heart failure management. Our team includes nurse practitioners, wound-certified nurses and collaborating physicians.
Can this help reduce hospitalizations?
Yes. Our goal is to help patients stay out of the hospital by preventing wound infections, complications and worsening of chronic conditions. We provide proactive, consistent wound care that supports long-term healing and reduces readmission risks.
Is this covered by insurance?
We accept Medicare. We work with each patient and their provider to confirm coverage before starting care.
How do I refer a patient?
Referring is simple. Providers can contact us here or call our referral line to submit patient information. We’ll coordinate care quickly and keep you updated throughout the process.
